Why an NCR backlog is a data model problem

A growing non-conformance queue looks like a capacity problem in the quality team. Usually it is not. It is that the deviation record cannot answer the question the reviewer has to answer, so every item needs a conversation.

Every plant with a quality system eventually has a non-conformance backlog. The usual response is to add reviewers, or to run a clearing exercise before an audit. Both work briefly. Neither addresses why the queue refills.

The reason is almost always the same. A reviewer cannot disposition a deviation without knowing what was affected, what caused it, and what has been done about the same thing before. If the record does not carry those three things, the reviewer has to reconstruct them, and reconstruction takes a conversation with the floor.

What a reviewer actually needs

Consider the decision being made. Someone has to decide whether affected material can be used, reworked or scrapped, and whether the process needs to change. That decision needs four things attached to the record.

Scope of impact. Which units, batches or lots share the condition. Not "the batch it was found in" but everything the same cause could have touched.

Point of origin. The operation, work centre, tool and shift, captured at the moment of the finding rather than inferred later from a date.

Prior occurrences. The same condition at the same operation, and what was decided last time. Without this, every deviation is treated as novel.

Current state of the material. Where the affected stock physically is, and whether it has already moved on or shipped.

Where the data model usually fails

Three failures account for most backlogs, and all three are structural rather than behavioural.

The first is a deviation attached to a batch rather than to an operation. A batch tells you when. An operation tells you where and how, which is what determines scope. Records keyed only to a batch force the reviewer to ask the floor what was actually happening.

The second is free-text description as the only classification. If the condition is captured as a sentence, there is no way to find prior occurrences except by reading. A controlled vocabulary for condition, cause and affected characteristic makes the history queryable, and it is worth the argument it takes to agree one.

The third is a disposition that does not record what happened to the material. The deviation closes, the stock moves, and the two facts live in different systems. Then a recall question cannot be answered from either one.

What changes when the record is right

When impact scope is derivable, the reviewer stops asking the floor to define it. When origin is captured at the station, clustering by tool or shift becomes a query rather than an investigation. When prior occurrences are attached, a recurring condition gets one decision applied consistently instead of six reviewers reaching six conclusions.

The queue does not disappear. It stops growing faster than it is worked, which is the actual problem.

A reasonable test

Before adding reviewers, take ten open deviations and ask what each reviewer needed that was not on the record. If the answer is consistently one of the four items above, the backlog is a modelling problem. Adding people to it will produce a slightly shorter queue at greater cost.
